Privacy and Security

LBDA is committed to protecting the privacy and personal information of individuals who contact the Association. Information collected by LBDA is for the sole purpose of furthering the Association’s mission, and will not be sold, rented or traded. The only personal information collected by the LBDA is that which is freely provided by contact through phone calls, mail, donations or our website. Personal information is stored in a secure environment and access only by authorized personnel with a legitimate and specific business purpose. By providing information, you consent to the collection, and understand that it may be used by Association to make further contact. To opt out of any or all methods of contact, follow the instructions on literature, if provided, or notify us by email (see below, by telephone or letter (see phone and address information below). Please note that discussions on group message boards are pubic information, and not protected under the privacy policies.

State Fundraising Notices

Although our financial report is always sent free to anyone requesting a copy, certain states require us to advise you that a copy of our financial report is available from them.

The Lewy Body Dementia Association (LBDA) is a 501(c)(3) nonprofit organization dedicated to raising awareness of the Lewy body dementias (LBD), supporting patients, their families and caregivers, and promoting scientific advances.

Maryland
Copies of documents and information submitted by LBDA are available for the cost of copies and postage from the Secretary of State, Statehouse, Annapolis, MD 21401 or by calling (410) 974-5534.

Mississippi
The official registration and financial information of LBDA may be obtained from the Mississippi Secretary of State’s office by calling 1-888-236-6167.

New Jersey
Information filed with the Attorney General concerning this charitable solicitation may be obtained from the Attorney General of the State of New Jersey by calling (973) 504-6215.

New York
New York residents may obtain a copy of LBDA’s annual report by writing to the Office of the Attorney General, Department of Law, Charities Bureau, 120 Broadway, New York, NY 10271.

Pennsylvania
The official registration and financial information of LBDA may be obtained from the Pennsylvania Department of State by calling toll-free, within Pennsylvania, 1-800-732-0999.

Virginia
